What we do
A full Medicare home health suite, under one plan of care.
Skilled nursing, therapy, wound care, infusion, and social work, all under one plan of care, delivered by clinicians who actually talk to each other. No handoffs between departments. No “that's not my job” gaps.
Skilled Nursing
RN- and LPN-led care in the home: medications, wound checks, IV therapy, post-surgical recovery, and chronic disease monitoring under a physician's plan.
Learn morePhysical Therapy
Getting back to mobility: strength, balance, walking, stairs. The bigger gross-motor pieces that decide whether someone stays safe at home.
Learn moreOccupational Therapy
Activities of daily living: being able to safely shower, dress, cook, use the bathroom. The everyday-life rebuild that PT alone can't cover.
Learn moreSpeech Therapy
Treatment for swallowing, communication, and cognition after stroke, TBI, or neurodegenerative disease, delivered by licensed SLPs in the home.
Learn moreWound Care
Certified wound care nurses treating pressure injuries, surgical wounds, diabetic ulcers, and complex non-healing wounds, in the home, on Medicare's clock.
Learn moreInfusion Therapy
Home IV antibiotics, hydration, pain management, and biologic infusions, covered under the home health benefit and safer than another hospital stay.
Learn moreMedical Social Work
Licensed MSWs coordinating community resources, caregiver support, benefits navigation, and psychosocial assessment when a medical episode disrupts a household.
Learn moreRespiratory Therapy
COPD, post-pneumonia, and ventilator-dependent care delivered by respiratory therapists in partnership with the patient's pulmonologist.
Learn moreHome Health Aide
Medicare-covered HHA support alongside a skilled plan of care: bathing, grooming, feeding, and ambulation help from certified aides.
Learn moreWhy Freedom
Home health built around the patient, not the paperwork.
Medicare home health is supposed to be a clinical visit at home, not an intake call followed by silence. The difference is in how the agency is run before the clinician ever walks through your door.
Continuity of care, by design.
We prioritize continuity: aiming for the same clinician walking your door on visit one and on visit two whenever scheduling allows. The office knows who's coming, and so do you. That's how home health is supposed to feel.
Next-day start for your traditional Medicare patients.
When the referral comes in, we run your benefits, confirm homebound status, and call you with the welcome call, usually getting the first visit on the calendar for the next day. Medicare Advantage takes longer because of prior authorization. We'll tell you exactly where you stand once we know your plan.
When you also need everyday help, we hand off warmly.
Medicare home health covers the skilled side: nursing, therapy, wound care. When you also need help with bathing, cooking, or transportation, we'll warm-hand you to our sister company #1 Home Care Services. No cold call, no surprise charge.
A real home safety look on the first visit.
The starter visit isn't just therapy. The clinician walks through your home and flags broken furniture, loose rugs, missing grab bars, anything that's going to put your loved one at risk. Whatever they see comes back to you, with a recommendation.
Our Promise
Every clinician Freedom sends into a home is one we'd trust with our own family.
That's not marketing. It's the hiring bar. Background checks, license verification, clinical references, in-person interviews. The same standard whether they're seeing one patient or fifty.
