Complex wounds heal faster when the dressing changes happen on the right schedule, the wound is photographed and tracked, and the clinician knows what they're looking at. Our wound-certified nurses bring the same protocols a wound clinic would run, minus the trip into the clinic and the infection risk that comes with it.

We coordinate directly with the referring physician on the wound care plan, document progress visit-to-visit, and adjust dressings, debridement, and topical therapy as the wound responds. When a patient also needs IV antibiotics for the wound, we run that as part of the same plan of care.

How to start

From referral to first visit.

    A physician, hospital case manager, or family member sends us the referral. The clock starts the moment it lands.

  2. 2

    Our intake team verifies Medicare benefits the same day and confirms homebound status. Traditional Medicare moves immediately. Medicare Advantage takes a little longer because of prior authorization.

  3. 3

    We complete the welcome call and coordinate scheduling for the start-of-care visit, usually the next day for traditional Medicare patients.

  4. 4

    The clinician completes a full evaluation, starts care, and walks the home for fall risks. By the time they leave, the rest of the schedule is on the calendar.
