Medical social work is the part of home health that handles everything outside the clinical visit but still squarely on the family's plate: figuring out community resources, navigating Medicare and Medicaid, lining up caregiver support, and dealing with the psychosocial weight of a hard diagnosis. Our licensed MSWs assess all of it and make the calls so the family doesn't have to.

We don't replace family decision-making. We give families the information and the connections they need to make a decision they're confident in, and we coordinate with the rest of the clinical team so the social and medical pieces of the plan don't drift apart.

How to start

From referral to first visit.

    A physician, hospital case manager, or family member sends us the referral. The clock starts the moment it lands.

  2. 2

    Our intake team verifies Medicare benefits the same day and confirms homebound status. Traditional Medicare moves immediately. Medicare Advantage takes a little longer because of prior authorization.

  3. 3

    We complete the welcome call and coordinate scheduling for the start-of-care visit, usually the next day for traditional Medicare patients.

  4. 4

    The clinician completes a full evaluation, starts care, and walks the home for fall risks. By the time they leave, the rest of the schedule is on the calendar.
